Why is basal insulin not running on a t:slim pump used with Dexcom G7?
Answer
Dexcom G7 reads glucose; basal insulin is delivered by the insulin pump (such as a t:slim X2), not by the G7 sensor itself. If highs continue unless you bolus, check that the pump’s active Personal Profile has a basal schedule selected—then treat persistent highs as a possible infusion-site or delivery problem and follow your clinician’s sick-day/ketone guidance.

Follow these steps
Separate CGM from pump delivery
Remember Dexcom G7 reports glucose; it does not deliver basal insulin. Confirm the question is about pump basal settings or infusion delivery, not a G7 “basal rate” menu.
You should see: You are troubleshooting the pump profile/infusion path rather than expecting the G7 to dose insulin.
Confirm the active Personal Profile and basal schedule
On the t:slim X2, open the active Personal Profile and verify a basal schedule is programmed and that the correct profile is selected. Correct empty or zero basal segments only using values your clinician prescribed.
You should see: The selected profile shows an intended basal schedule for the current time of day.
Check for delivery problems when settings look correct
If basal settings look right but glucose keeps rising, inspect the infusion set/cartridge path for occlusion or dislodgement per the pump’s instructions, and check ketones if you have supplies and your care plan says to.
You should see: You either find a delivery issue to fix or have ketone/high-glucose information for your care team.
Use clinical support for ongoing highs
For persistent hyperglycemia, ketones, or uncertainty about doses, follow your prescribed sick-day rules and contact your diabetes clinician or emergency services as directed—do not wait on app restarts alone.
You should see: You have a safe next action from your care plan rather than only device menu changes.

Extra tips
What to look for: Active Personal Profile on the pump; basal schedule entries; pump alerts; ketone results if highs persist
Before you change anything: This is diabetes device troubleshooting, not medical advice—follow your clinician’s plan for highs and ketones. Do not change insulin doses based only on a help article. Have backup insulin and supplies available per your care plan. Use only official pump/CGM menus; ignore strangers offering remote “resets.”
